We are looking for RN/LVN Care Managers in many areas. This position is part-time/full time, weekly in home visits for an assigned caseload. No weekends, no on call and the ability to make your own schedule. This position requires you have your own transportation, and ability to use a computer with secure internet access. You must be comfortable working with little to no supervision. You will be case managing patients for their insurance company, providing them with tools and resources to prevent hospitalizations.

Must have at least 2 years of experience with geriatric/ home healthcare experience preferred. Must be able to pass a drug screening and background check.
